for PSPX but can be tried on the earlier or later version.
Open a canvas 400x400, transparent.
Selection tool/ circle. Place a selection circle approximately
250x250 on the canvas. Add a raster layer and name it "button
base" and flood fill with any color or gradient you desire.
Selection/ Modify/ Contract by 20. Add a raster layer and name it
"button ring", flood fill with color you desire.
Effect/ 3D Effects/ Inner Bevel. Settings below.

Deselect.
Selection tool/ circle. Place a selection circle approximately
40x40. Add a raster layer and flood fill with black. Deselect.
Duplicate the black ellipse. Place the copy on the opposite side of
the other ellipse. Merge the layer down. Go to the layer palette and
change the layer name to "button holes".
Make it selected then go to the layer palette and click on the the
button base layer. Hit delete. Delete the button holes
(black ellipses).

Make the button base layer selected and apply inner bevel.
Paint brush/ round/ size 14. Paint brush the button hole with any
color or textured pattern. See screen shot.

Preset shape, rectangle/ color of your choice or textured pattern.
Place a rectangle connecting the button holes. Convert to raster
layer. See screen shot below.

Merge the layer down. Make the layer selected. Effects/ 3D effects/
cutout. Settings below.

Lighten/ Darken tool. Settings below. Lighten the center part of the
thread or string that connects the button holes.

Merge the layers visible. Save your image.
